On my busiest clinic days, I can usually predict which antenatal register will keep me on my toes — it's almost always the one with the youngest mums. Why a regional retrospective matters beyond its borders
Retrospective studies are not glamorous. They do not give us the clean causal architecture of a randomised trial. But what they do give us — especially when they come from busy tertiary centres in Western India — is a mirror. They reflect the actual mix of patients walking through our doors: the anaemic nineteen-year-old in her third trimester who has travelled two hours for a check-up, the thirty-two-year-old professional managing gestational diabetes alongside a board meeting, and the very different care pathways each one needs. The Cureus analysis joins a growing chorus of clinician-led work asking us to stop treating "age" as a single risk flag and start treating it as a lens that refracts everything else — nutrition, socioeconomic support, birth preparedness, and access to skilled attendance.

I am always cautious about extrapolating from a single-centre retrospective to national policy. But I am equally cautious about ignoring what such studies tell us about the texture of risk. What I would want every practitioner reading this to take back to their OPD on Monday morning is simple. If you work in a district hospital, a PHC, or a community clinic and your teenage pregnancy numbers have not budged, do not assume the next study will save you. The questions to ask yourself are the ones this Western India cohort implicitly raises: Are our antenatal visits longer for adolescent mothers, or shorter? Are we screening for anaemia, hypertensive disorders, and preterm labour risk more proactively in the under-twenty group? Are we linking them to nutrition support and postnatal contraception before discharge, or are we letting them slip back into the same environment that brought them to us? The data will keep arriving. The bedside decisions are ours to make now.
